The Warrick County Health Department can still provide immunizations for your children. We just have some new guidelines and new ways to provide this service.

-If you have private insurance,it is imperative that the insured contact their insurance company to verify coverage for immunizations. Please be aware, if the insurance provider does not cover vaccinations you will receive a bill for the vaccines your child received from a third party billing company and  vaccines can be very costly. You will need to bring your insurance card at the time of your appointment so insurance verification can be obtained.

-For those with no insurance, those on Medicaid or Hoosier Healthwise or those whose insurance does not cover immunizations everything has stayed the same.  We will need a copy of the Medicaid card.

-As always you must bring your immunization card/record!

-If you do not have your insurance card, Medicaid card and immunization record you will be asked to reschedule and return when you have those available.

School Entry Immunization Requirements

Below are the number of doses and each vaccine required for school entry.

3-5 years old:               3 Hep B (Hepatitis B)

                                      4 DTaP (Diptheria, Tetanus & Pertussis)

                                      3 Polio (Inactivated Polio)

                                      1 MMR ( Measles, Mumps & Rubella)

Kindergarten to            3Hep B     

 2nd grade                     5 DTaP      2 MMR

                                      4 Polio       2 Varicella

Grades 3 to 5               3 Hep B

                                      5 DTaP      2 MMR

                                      4 Polio       1 Varicella

Grades 6 to12              3 Hep B     2 MMR

                                      5 DTaP      2 Varicella

                                      4 Polio       1 Tdap (Tetanus & Pertussis)

                                                          1 MCV (Meningococcal)      

Varicella: Physician documentation of disease history, including month and year is proof of immunity for children entering preschool, kindergarten and 1st grade. A signed statement from the parent/guardian indicating history of disease, including month and year is required for children in grades 2 - 12.
